$80/month per adult (18 years and above)
$20/month per child (17 and under)
This fee is paid every month, regardless of how often you visit the office. You won't pay more if you come in multiple times, nor will you pay less if you don't come in at all.

What is included:
Unlimited visits, same-day appointments, sports physicals, DOT physicals, referrals to specialists if needed, and telehealth capabilities.
Significantly discounted (Cash Pay) rates on laboratory testing, medications, procedures, and radiology imaging when needed.
Ability to use your current insurance on medications, laboratory testing, and radiology if you choose.
To become a new patient, we require an advance payment of $240.00 for an adult, $60 for a child for the first three months. After that, your next payment will be due in the fourth month. Starting from the fourth month, we'll automatically deduct your payments from your bank account, debit/credit card, or Health Savings (HSA) or Flexible Savings Account (FSA)
